Job Description

An exciting opportunity has arisen for a Band 4 Assistant Practitioner to be based within the Carmarthenshire Community Nursing Team.

The successful applicant will be assigned to an individual Community Nursing Team within the county and location will be discussed at interview.

The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader. The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

Carmarthenshire Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.

This is a part time position worked over a 7-day period including weekends and bank holidays. The post holders will need the ability to travel between sites in a timely manner.

Duties will include wound care/management, administration of medication, venepuncture, catheterisation and continence promotion, holistic re-assessment, recording of observations and health promotion. The ability to recognise an unwell patient and how to escalate care accordingly is essential, as is the ability to effectively manage your daily caseload.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
